Getting a CHW
For current RI Anchor Card Medicaid recipients
Community Health Worker Services (CHW) must be recommended by a licensed practitioner of the healing arts to begin services.
This could be as easy as the individuals seeking CHW services to ask their licensed practitioner for a statement in their electronic file, or written statement.

We must receive this statement which can be handed or delivered to us.
CHW SERVICES WILL BEGIN AFTER this is received.
LICENSED PRACTITIONERS OF THE HEALING ARTS INCLUDE:
All Physicians including specialty Physicians
Physician Assistants (PA)
Advanced Practice Registered Nurse (APRN)
Registered Nurse (RN)
Licensed Practical Nurse (LPN)
Certified Nurse Midwife (CNM)
Dentist
Licensed Dental Hygienist (LDH)
Podiatrist (DPM)
Licensed Chemical Dependency Counselors (LCDC)
Psychologist
Occupational Therapist (OTRL)
Licensed Marriage and Family Therapist (LMFT)
Licensed Clinical Social Worker (LCSW)
Licensed Independent Clinical Social Worker (LICSW)
Licensed Mental Health Counselor (LMHC)
Certified Professional Midwives
Pharmacist
At this time, we are unable to bill for services for members with Medicare or Private Insurance.
If you have any questions or are a self pay client, contact us for more information.
CHW services are considered medically necessary for beneficiaries:
1. With one or more chronic health (including behavioral health) conditions;
2. Who are at risk for a chronic health condition;
and/or
3. Who face barriers meeting their health or health-related social needs are eligible to receive services from a CHW.
The recommendation for CHW services does not need to include the health conditions of the member, since a screening will be performed at the time of intake.
